At Mid Devon Healthcare Primary Care Network, our therapist led OT/Elderly Care and Frailty Service is here to help people aged 65 and over and anyone showing early signs of frailty to stay well, independent, and connected.
We understand that ageing can bring changes in health, memory, and mobility. Our team works closely with you, your family or carers, and other health and social care professionals to provide personalised, proactive care. Our aim is to help you live well for longer, prevent avoidable illness and make sure you have the right support at the right time.
What We Offer
- Early assessment and support for concerns about memory or cognitive changes.
- Annual dementia reviews to include a holistic review with patients and their carers, medication checks, and access to local support.
- Proactive health and wellbeing reviews for people over 65 or those with early signs of frailty.
- Holistic, person-centred assessments that look at your physical, emotional, and social wellbeing.
- Personalised care plans focused on maintaining independence and quality of life.
- Advice and guidance for carers, including help to access local and national support services.
- Falls prevention and mobility advice to help keep you active and safe at home.
- Treatment escalation and advance care planning, helping you to make informed choices about your future care and preferences.
- Links with community, social care, and voluntary organisations to ensure joined-up, co-ordinated support.
Meet the Team
Our Frailty Service is led by Nikki, Elderly Care/Frailty Team Lead, and Jess, Frailty Coordinator.
Together, we work as part of the wider PCN team to support you and your carers through holistic assessment, proactive care planning, and access to the right local services.
How to Access Support
If you’re worried about frailty, memory changes, or a decline in wellbeing — for yourself or someone you care for — please contact your GP practice.
You can ask to be referred to the PCN OT/Frailty Team, who will be happy to offer advice, support, or arrange an assessment.
